Scenes in this film were filmed at the old Medfield State Hospital in Medfield, Massachusetts. Formerly, scenes were going to be shot at the old Worcester State Hospital, but the demolition of the surrounding buildings could have interrupted the shooting which was unfeasible. So, for the story's island setting Peddocks Island was used and for the lighthouse scenes East Point, in Nahant, Massachusetts was used.
